The season follows both Scofield inside Sona, and Burrows on the outside. Sona is a prison that has been run by the inmates and guarded only from the outside since a riot the year before. Burrows is quickly contacted by The Company who have kidnapped his son LJ and Sara Tancredi, the woman Scofield loves, and is told that they want Scofield to break James Whistler out of Sona. The season follows Scofield and Whistler's trials in formulating an escape plan, as Burrows deals with The Company's sexy and deadly representative.
When Scofield fails to meet their deadline and Burrows attempts a double-cross, Company operative Gretchen Morgan claims to have personally beheaded Tancredi as a warning. As the season ends, the pair manage to escape, leaving behind several accomplices. LJ is traded for Whistler, and Scofield is out for revenge against Gretchen for Tancredi's "death".

The Unit is a U.S. Army organization similar to Delta Force. As with Delta Force, its members come from the United States Army and appear to be primarily tasked with counterterrorism. The unit's official cover is as the "303rd Logistical Studies Group". In the third season's premier, an onscreen read-out specifically identifies the unit as "ALPHA Team 1st Special Actions Group".
The Unit is based at a fictional army post, "Fort Griffith". The location of Fort Griffith is never explicitly stated, but in Episode 303 "Always Kiss Them Goodbye", the storyline suggests that it is northwest of Pine Bluff, Arkansas and west/southwest of St. Louis, Missouri, which would place it in the vicinity of the real-life Fort Leonard Wood. The common sight of the Missouri license plate supports Fort Griffith being placed in Missouri, as well as a flyer in Episode 206 "Old Home Week", handed out at an anti-war rally at the Fort Griffith gates, inviting fellow protesters to a seminar in Greenwood, Missouri.

The series begins as a seemingly ordinary group of people gradually become aware that they have special abilities. Events illustrate their reactions to these powers, and how the discovery affects their personal and professional lives. At the same time, several ordinary individuals are investigating the origins and extent of these abilities. Mohinder Suresh, a geneticist, continues his late father's research into the biological source of the change, while Noah Bennet represents a secret organization known only as the "Company".
